New Hampshire State Health Improvement Plan 2013-2020,Charting a Course to Improve the Health of New Hampshire

The New Hampshire State Health Improvement Plan (NH SHIP) was developed with input from partners from the diverse sectors, agencies and organizations that address population health in New Hampshire. It identifies 10 priority areas for improvement with measurable objectives and targets for health outcomes; areas for needed attention in public health capacity; and, recommendations for evidence-based interventions and actions. It includes measurable objectives, recommended strategies for improvement, and performance measures with time-framed targets for each priority. The NH SHIP priorities and objectives are intended to provide support, guidance, and focus for public health activities throughout the state. The NH SHIP is the state’s public health roadmap, providing evidence-based strategies to guide the direction of many of our actions. The NH SHIP objectives are our destination; reaching them will mean that we have significantly improved the health of our people.

The 2011 Snapshot of New Hampshire's Public Health Regions, Counties, and the Cities of Manchester and Nashua (4.99MB) is a companion document to the 2011 New Hampshire State Health Profile. Our aim with this report is to provide health profiles of New Hampshire’s regions to assist community leaders to focus their work to improve the public’s health at a local and regional level. These profiles highlight 30 indicators selected to best describe the health of the people in New Hampshire communities.

The 2011 New Hampshire State Health Profile Report highlights 38 indicators to describe the health of the people in New Hampshire. It is intended to be used to set state and regional priorities to improve public's health in New Hampshire.

PDF Forms: New Hampshire court forms are being presented on the Internet in Adobe's Portable Document Format (PDF). We are currently using Adobe Acrobat 11 Professional to create these forms and are saving to level Adobe Reader X. The Adobe format has been chosen because of its near universal adoption for providing electronic copies of forms where the content and presentation of the document requires consistency. That consistency is not reliably provided using standard web forms. A free viewer is available from Adobe, so this format imposes little financial burden on the user. Forms not available in Word: We have chosen not to provide forms in any word processing format because the ability to 'lockdown' the form into a standard look and feel is limited at best. The selection of which word processing solution to use to fill out the form can cause inconsistencies. Even the selection of which printer to use to print the form can lead to results that are inconsistent and impractical to process. Users of completed forms (court staff, judges, parties, etc.) expect to see specific content in consistent locations on the form. When that content shifts as a result of printer and word processor anomalies, then the value of standardized forms is diminished.

Filling out Forms: All of these forms can be printed as blank forms and filled in manually if required. All these forms can be saved to local electronic media in their blank state. Users are cautioned that when they pre-print a form for later use or save a copy of a form to local media, they need to make sure that the form has not changed since it was last printed or saved locally. It is the users responsibility to check the court's website for updated forms to avoid using obsolete and outdated forms.

Most NHJB forms are also capable of being completed electronically through Adobe Reader, using the TAB key to navigate the form from field to field and keying data into each field. With Adobe Reader OLDER than XI, you cannot save the completed (or partially completed) form to electronic media. If you are using Adobe Reader XI or NEWER you can complete (or partially complete) a form and save it. You can print the form with any version when complete. Forms that are not yet in a fillable format will be placed in that format in the future.
